• Our customer service team uses this document when onboarding a new client and setting up their profile or to update a current client's information in our system

PureOHS Employer Confidentiality Statement

• PureOHS is our online employer portal where employers can access records, pull reports, etc. This statement must be completed by the employer when signing up for the portal

Our team partners with more than 3,000 clinics across the United States for some services, including drug testing, DOT physicals, and pre-employment assessments. Using our electronic authorization portal, we can set an appointment at a convenient location for your candidate—no matter where they live.

Virtual work injury triage

This virtual service option directs injured employees to the most appropriate place of care, which could be the workplace (if the injury can be handled with basic first aid), an occupational health clinic, an urgent care or walk-in clinic, or the emergency department.

Before accessing this service, employers must complete a virtual work injury triage form to set up a profile.

Employer’s Request for Respirator Clearance Form

• The employer completes this form to indicate which respirator the employee will be wearing, under what conditions, and for how long
