Our health coaches, registered dietitians, and chefs, along with our lifestyle medicine doctors recognized by the American College of Lifestyle Medicine, are here to empower your journey to better health. Health coaches
Focused on your individual goals, our health and wellness coaches are board certified through the Wellcoaches School of Coaching.

Registered dietitians
From allergies to food intolerances, our dietitians will work closely with you, providing tailored dietary guidance.

Lifestyle medicine doctors
Our doctors will partner with you to develop a lifestyle plan that’s right for you.

Chefs
Our team of chefs is ready to share easy tips and tricks to help you add more healthy, plant-based foods to your meals.

How it works

Begin by meeting with your lifestyle medicine doctor who is your partner throughout your journey. Using MyChart, you can request an in-person or virtual consultation.

Initial consultation
Your doctor will perform a basic health/behavior assessment.

Setting goals
Based on your health goals, you doctor will begin to design a lifestyle medicine plan that you can track together.

Lifestyle medicine programs
Over the course of three-to-six months, you will meet one-on-one with health coaches, dietitians, and lifestyle medicine doctors (as needed). Our team will recommend programs ranging from shared medical appointments (SMAs) to webinars with dietitians to chef-led cooking classes.

Program completion
Once again, your doctor will perform a basic health/behavior assessment to gauge your progress and determine next steps.
