To view your child’s medical information and message their providers, you must have them added to your MyChart patient portal through Proxy Access. Parents/legal guardians can request access to children between the ages of 0-17 in MyChart or by calling MyChart Customer Support at 877.308.5083.

Corewell Health patients between the ages of 12-17 can access their own medical records through MyChart, using their own username and password. This can be established by contacting MyChart Customer Support at 877.308.5083 or request with a provider. The adolescent must be present on the phone or in person when creating their account.

All other information must be obtained at the provider’s office or through our medical records department.

How to request minor proxy access

Follow the steps below to submit a request in MyChart:

  1. Log in to MyChart
  2. Click on the Menu button
  3. Under Sharing, click "Request Minor Proxy Access"
  4. Fill out the form with the minor’s information and your information
  5. Submit the request
  6. You will receive a MyChart message once your request has been reviewed

Once you have access to a minor (ages 0-11)

This type of access is for parents/guardians of minor children (age 0-11) and grants full access to the minor's record.

Once you have access to an adolescent (ages 12-17)

Parents/guardians of adolescents have the right to Standard Adolescent Access. This can be established by a Corewell Health provider’s office, Customer Support, or minor proxy request in MyChart:

  • This access includes upcoming appointments, scheduling new appointments, messaging to and from providers, allergies, and immunizations
  • Parents/guardians have the right to request additional medical information, such as test results, from our medical records department or their provider’s office

The reason access to your adolescent’s MyChart account is limited is due to limitations of our electronic medical records system (EMR). Because of Michigan law, when an adolescent turns twelve (12) years of age, certain medical information in their medical record at Corewell Health is protected, and the adolescent does not need parental consent to receive certain services (e.g., services related to pregnancy, sexually transmitted infections, substance abuse, and behavioral health). The relevant Michigan laws are MCL 333.5127, MCL 333.5133, and MCL 722.623.
