Sharing your own information
Grant access to your MyChart account
MyChart proxy access allows you to access the medical records of your family members and others you care for, with their permission:
- You must be at least 18 years old to have proxy access to another person’s medical record
- Parents/legal guardians can request access to children between the ages of 0-17
To give someone access to your medical record, grant access via a proxy invite through the Friends and Family Access page in MyChart:
- Log in to MyChart on your computer or in the mobile app
- Click the menu icon in the top left corner
- Under "Sharing," click "Sharing Hub". Alternatively, search "Sharing Hub," then click it.
- Under "Who do you want to share your health information with?" click "Family member, close friend, or caretaker"
- Choose how you want to share your health information
- Select “Invite friends or family”
- Enter the requested information and submit your request
- Ask the person you invited to log into their MyChart account and accept the proxy access request
Adult caregiver proxy access
The Adult Caregiver Proxy Access form exists to allow authorized caregivers — such as those in assisted living or adult foster care facilities — to securely access a patient’s MyChart account. This ensures they can manage appointments, view health information, and coordinate care on behalf of patients who need additional support.
Share records with health care team
You can share your medical information with anyone who provides care for you, even if they aren’t part of Corewell Health. This might be a doctor, chiropractor, physical therapist, dentist, or school nurse, for example. Access is temporary and includes only parts of your record.

Access a loved one’s records after a loss
We are very sorry for your loss. During this time, you may want access to medical information related to your loved one’s care. MyChart bereavement proxy access allows family members or representatives to request temporary access to relevant records, test results, and care details. Access is granted for six months, and requests are reviewed carefully to protect privacy, with documentation required.
