Rotation Application at Wayne

Elective rotation application

  • Applications must be submitted at least 30 days in advance
  • Applications are not accepted or processed June 1 – August 1 
  • Elective rotations are not offered in June, July, August and December
  • Complete an application form and attach the following for each rotation request
    • Letter from your medical school stating that you are a student in good standing
    • USMLE Step 1 or COMLEX Level 1 score report (USMLE below 220 or COMLEX below 440 will not be considered)
    • A current certificate of your liability insurance
    • Copy of photo identification - medical school ID card or driver’s license (photographs are not acceptable)
    • Documentation of TB test within past 12 months
    • Current record of your immunizations
    • Documentation of annual influenza vaccine for November through April rotations
