Rotation Application at Wayne
Elective rotation application
- Applications must be submitted at least 30 days in advance
- Applications are not accepted or processed June 1 – August 1
- Elective rotations are not offered in June, July, August and December
- Complete an application form and attach the following for each rotation request
- Letter from your medical school stating that you are a student in good standing
- USMLE Step 1 or COMLEX Level 1 score report (USMLE below 220 or COMLEX below 440 will not be considered)
- A current certificate of your liability insurance
- Copy of photo identification - medical school ID card or driver’s license (photographs are not acceptable)
- Documentation of TB test within past 12 months
- Current record of your immunizations
- Documentation of annual influenza vaccine for November through April rotations
