More Life Más Vida is designed to help you understand the status of your current health, your risk for diabetes and cardiovascular disease, and some simple behavior changes you can make to positively impact your well-being.
Our services
- Free health screenings
- Dedicated care team, including a registered nurse and community health worker
- Health education
- Physical activity and nutrition classes
Eligibility
- Age 18 or older
- Kent County resident
- Not previously diagnosed with diabetes, heart attack, or stroke
- Not pregnant or within six months postpartum
- Has limited access to health care
