Requesting medical records for a deceased patient: Contact the medical records department at the appropriate location. You may be asked to provide additional documents to obtain a copy of the records. If you had proxy access, are a family member, or were a legal guardian of someone who has passed away, you can request MyChart proxy access to view their medical records for up to six months after their passing.
