What is the PATCH program?

Our passion is to enable children to focus on life, the things they enjoy, and the people they love.

Corewell Health Children's PATCH is a pediatric palliative care program that provides comfort and support to children of all ages with life-limiting conditions. PATCH stands for Palliative Team and Children's Hospice. We serve those unborn through early adulthood. Our commitment is to provide compassionate and personalized care to children as well as practical and spiritual support to the entire family.

We focus on meeting the medical, spiritual, and emotional needs of the children and supporting everyone who cares for them. Our hope is that this will allow children and families to be families for as long as possible, making every moment together special.

The program includes:

  • Physical therapist for those children who may benefit from therapies that optimize function and reduce pain and distressing symptoms
  • A chaplain who provides emotional, spiritual, and bereavement support to your family
  • A board-certified hospice and palliative medicine pediatrician who provides expert symptom management and coordinates care with your child's doctors
  • Dietary guidance for your child (if needed)
  • Community education and support
  • A pediatric registered nurse who provides regularly scheduled visits and on-call availability, 24 hours a day, seven days a week
  • A home health aide who provides assistance with your child’s personal care and activities of daily living
  • A social worker who provides your entire family with counseling and support while assisting with community resources and memory-making
  • Volunteers who provide companionship to your child, which allows you time for yourself and your family

Frequently asked questions

Payment information

Corewell Health's PATCH program will never refuse care to children and families who are unable to pay for services. We receive payments through private health insurance companies, Medicaid, Children's Special Health Care Services, individual payments, grants, and generous donations.
