Our Health Information Management Department maintains the confidentiality of all medical records of Corewell Health patients.

You can request a copy of medical records for:

  • Yourself
  • Your child (under the age of 18)
  • An individual for whom you are their legal guardian

To request a copy of your medical records:

  • Print, complete, hand sign, and mail or fax the request to a Corewell Health site
  • Log into your MyChart account, select Menu then Request Medical Records

Birth certificates, affidavit of parentage, and death certificates

Corewell Health does not issue birth certificates, affidavit of parentage or death certificates. If you need a copy, you can request one from the State of Michigan, or the city or county clerk where the birth or death occurred.

Update your medical records

You have the right to request a change, amendment or correction to certain parts of your health information that Corewell Health maintains.

To request changes or corrections to your doctor's office medical record, contact your doctor's office. For your hospital medical record, please complete all sections on the Request to Amend Health Information and send to the HIM department by mail, email or fax, as indicated on the form.
