Residency and Fellowship Verification and Student Loan Information
For verification of residency, fellowship, or liability/malpractice insurance, please email your request along with a signed authorization to release information to residentverifications@corewellhealth.org.
All verifications are considered important and processed in the order in which they are received. The ideal turnaround time is five business days.

Electronic submissions (recommended):
You can now submit your PSLF application electronically by visiting the Federal Student Aid site. When submitting electronically, please submit to HRCompliance@corewellhealth.org.
The Corewell Health HR Compliance team typically completes requests within 10 business days. Please ensure that your PSLF form is filled out with the following information before sending it to HR Compliance. If your form is not filled out correctly, it will cause delays and a possible denial.
Paper submissions
You may submit a Paper PSLF form. If you are a current resident or fellow, please submit the form through the Corewell Health ServiceNow platform. If you are a graduated resident or fellow, please send your paper PSLF form to paige.parmelee@corewellhealth.org.
You can follow up on your PSLF request by calling the HR Service Center at 877.275.4711, Monday – Friday from 7:30 am to 4:30 pm.
