Rotation Application at Dearborn
Elective rotation application
- applications must be submitted at least 30 days in advance
- applications are not accepted or processed June 1 – August 1
- elective rotations are not offered in June, July, August and December
Complete an application form and attach the following for each rotation request, maximum two rotations
- letter from your medical school stating that you are a student in good standing
- USMLE Step 1 or COMLEX Level 1 score report (USMLE below 220 or COMLEX below 440 will not be considered)
- a current certificate of your liability insurance
- copy of photo identification - medical school ID card or driver’s license (photographs are not acceptable)
- documentation of TB test within past 12 months
- current record of your immunizations
- documentation of annual influenza vaccine for November through April rotations
Only two applications per student will be accepted
Submit completed application and all seven attachments, for each rotation requested, to the Medical Education Office at
