Patients can expect compassionate, extraordinary care provided by a professional medical staff member. Our practice has partnered with other specialists that operate within the same location. The specialists include an orthopedic surgeon, Bariatric surgeon as well as a Gastroenterologist. Before you arrive: New patients should complete paperwork mailed from our clinic prior to their appointment. We ask patients to arrive 15 minutes prior to their appointment to be pre-registered at check in. Arrive with valid ID and Insurance card(s). Patients who require an authorization/referral for insurance purposes will need to do the following: Request an authorization/referral from their primary care physician or referring physician. Call our office one week prior to their scheduled appointment to insure we have received the required referral. Parking information: Park in lot 1. Handicap parking available close to the main hospital entrance. Once you are parked, enter through the main hospital entrance and take the first elevator to your right (across from the service desk) up to level 2 and suite 200 is immediately to your left.
