Primary care in the comfort of your home

Home Based Primary Care delivers personalized health care in the comfort of your home. Designed for patients with advanced chronic conditions such as diabetes, chronic obstructive pulmonary disease (COPD), and hypertension who face challenges getting to a traditional primary care office. Our program ensures you receive the care you need — where you need it most.

Your care team and services

Our dedicated health care team works together to bring care right to your home. Your personalized plan will include support from a primary care doctor, advanced practice provider, and a registered nurse care manager. Support as needed is also available from a social worker and medical assistant.

We provide services designed to keep you healthy and at home, including:

  • Managing your primary care needs
  • Coordinating your care across providers and specialists
  • Offering ongoing support to prevent hospitalizations and improve health outcomes

How do I qualify?

  • Qualification is based on medical need and difficulty accessing a traditional primary care office
  • Home Based Primary Care is for Priority Health insured patients (Medicare, Medicaid, or commercial plans)
  • Patients must live within the service area: Kent, Newaygo, and Ottawa counties, and portions of Allegan, Barry, Ionia, Mecosta, Montcalm, and Muskegon counties
  • If your condition improves, you may no longer qualify for Home Based Primary Care. In this situation, we would assist you in finding the services you need in the community
