Corewell Health is committed to continually improving the health of the communities it serves.
The mission of Corewell Health is to improve health, instill humanity, and inspire hope. To fulfill this mission, the Healthier Communities initiative focuses on reaching the highest level of health equity possible with the people, neighborhoods, and regions we serve.
This can be accomplished by every regional community working to:
- Conduct a Community Health Needs Assessment (CHNA)
- Develop an implementation strategy that communicates how identified health needs will be addressed
- Complete an impact report that details the measurable results of the implementation strategy
